故事发生在明朝年间，叶常绿（黄德斌 饰）是朝廷刽子手，因为奉命斩首了恩公吕大人而心怀愧疚。一次偶然之中，叶常绿邂逅了吕大人身怀六甲的小妾阮素心（姚子羚 饰），将其救下，藏在京城第一稳婆花蕊红（邵美琪 饰）家中待产，并威胁后者不准将此事泄露出去。
